The vase is signed Holmegaard Pl 1961 and has modelnumber 15734.

The color is also called akva. This is the more light and blue version on the color.
The heart vase was designed by Per Lütken in 1955 and taken out of production in 1974. The design is relaunched in 1985 as part of the Evergreen series. The original name is the Menuet.
